Support Coordinators and Care Managers have their Diploma of Community Services Case Management, diplomas or bachelors in Social Work, and/or Advanced diploma of Community Sector Management.
Our support coordinators will assist you in walking you through the NDIS, understanding and implementing your plan, finding and engaging the providers of your choice and assist you with plan reviews. Our goal is to support you to implement your plan to have the best services and always ensure you have choice and control over your plan. We want you to achieve your goals and also increase your capacity to be able to manage your future plans and engage more efficiently with your service providers.

We can walk you through the applications, assist you in understanding and gathering the documentation you need and to find government and mainstream supports in your community that can support you to lead a better life and increase your quality of life.
We aim to help you achieve wellness and autonomy which include gauging care, budgeting, planning, facilitating, evaluating participants’ needs, optimizing human services, and monitoring your health and wellbeing. These services are private and are not part of/ funded by the NDIS or Home Care Packages. But we can assist you in applying for a plan in one of these programs/ schemes.
ESSA’s Pre- and post-employment job support is designed to help individuals or a group of individuals to access and maintain employment, starting with training and developing professional skills, practices, routines to take up employment opportunities, interview skills, volunteering, resume building etc.
We help you developing critical skills necessary to thrive in the modern economy. We offer opportunities of in-house and out of the house training, flexible working hours and link you with professional networks and communicate with employers and maintain your job.
We can also offer other supports to make it easier to manage your new job, such as training for public transport to go to your new job, induction or training assistance if needed, enhancing social interaction and inclusion in the professional environment.
Support connection and coordination assistance by linking our support to other supportive community services and programs such as: accommodations or tenancy procedures, budgeting, mentoring, peer support, establishing supportive networks in your community to avoid crisis and assist in conflict resolution with decision making, planning, choice, control and plan review.
Innovative community participation is a program that aims to support NDIS participants with the opportunity to take part in special activities that are not covered by other community participation categories to build skills to actively participate in their community, expand opportunities for community participation and/or employment.
We focus on resilience, wellbeing and personal growth to successfully connect to the community, each service is tailored to best suit your goals. Full community engagement is the goal, and we want to join all parts of your community life to live it to the fullest. This service links community activities, social interactions, employment/ study and much more.
Understanding your goals and working with you and your family or carers to put in place supports to develop everyday skills with daily support like: daily life skills, routines to gain knowledge and build confidence to enhance independence, public transport training to use public transport independently, travel or holidays, support and development skills for community participation, social and recreational participation, personal hygiene, toileting, personal care, bill paying, money handling, household tasks, cooking routines or preparing a healthy meal, time management, problem solving or unique skill, training, development activities to improve your skills and reach your goals.
Our team also crafts a daily exercise and meditation plan, or includes relaxing techniques to keep your body and mind in perfect shape. We implement strategies that include one to one training, managing the funding of supports, communicating with service providers and building support networks.
Assistance to increase your independence and functional skills according to your age and circumstances through supports that meet your needs and goals in a less intrusive manner. Personal care support (you choose your level of assistance or supervision needed) with personal tasks of daily life, things you do every day.
We can help with personal hygiene activities including showering, bathing, oral hygiene, dressing and grooming, toileting, eating, drinking, mobility to get in and out of bed, attending appointments, use of aid and appliances, hearing and communication devices, mobility and transfering.
Accompanying you to develop skills and build capacity to independently travel using public transport or a support worker can take you in their own vehicle for your travel needs.
Our support consists of personal transport training to use public transport, related aids or providing transport means. Transport is fundamental to participation in community, social, economic and daily life activities and we aim to reduce barriers to this.
We can also organise specialist transport arrangements and alternative forms of transport such as taxis, maxi-taxis, rideshares, community transport and travel assistance with an assigned support worker that will drive you where you want to go. We ensure that the support drivers that we source for you are well trained and are properly briefed about your needs, requirements and preferences.
This support is to empower and support you to access your community services and activities. We can support you with activities and commitments you have such as going to your work, school, appointments etc. But we also want you to enjoy life and do activities in the community that you love.
We can arrange support workers to drive you in their cars or go together on public transport to train you to be confident, as well as accompany you to your places of choice and do activities with you.
We also organise group activities and outings for you to meet more people around your community and increase your social participation. Some examples of places for community participation are: shopping centres, social events, medical appointments, visit family or friends, pick-up or drop-off things, travel requirements for requested locations such as from your place of work, study, community participation, workshops, festivals, events, and more.
We organise for participants that are in areas close together that have similar likes and preferences to meet and do activities they enjoy in a group.
We want to include everyone in the community and to be a way for you to meet new people from a range of different backgrounds, ages, capabilities, genders and more. We can organise community outings to go to places such as the cinema, festivals, zoos, concerts and much more.
We are specialists in crafts and boardgames and we believe that everyone can learn from someone, and everyone can teach and share their knowlede. We organise activities in community centres or neighbourhood houses to do craft activities together, do work with recycled materials, play board games or even do cooking and other types of classes together. We are open and we love participants input. If you want to do an activity, let us know and we will make it happen!
We can assist you to find, purchase and deliver assistive products for your personal care and safety. Many NDIS Plans include consumables funding so depending on your plan and your needs, you might be able to have these assistive technologies fully or partially funded by your NDIS plan.
We can work with you and your support network (such as family, carers, support coordinators and allied health specialists) to determine what would be best for you. Assistive producs for personal care and safety include: bathroom equiptment, beds, rails, consumables, pressure mattresses and more. We can also organise rentals or purchase of the equiptment, depending on what you need.
These products include aids and equipment to assist you to take part in competitive and non-competitive sports and other recreational activities. These can include bycycles and tricycles, accessories and products that enable you to take part in sports, Adapted toys/Switch toys/Adapted Game Interface for gaming in the digital world, as well as repairs for recreational equiptment you already have.
IMPORTANT: These products are generally not funded or not fundded fully by the NDIA without sufficient justification, meaning that there needs to be a reason as to why you need it and why you cannot purchase it without NDIS funds. The NDIA is constantly reviewing their approach to equiptment for recreation.

These products can assist you in doing tasks at home and tasks that are part of your daily routine and to assist you and your carers at home. The equiptment can support you in areas such as food preparation, eating, household cleaning and other home activities.
All of the services above can be scheduled around your routine. All prices are according to the NDIS Price Guide and are regularly updated. If you have a preferred language other than English, we can assist you with interpreting services or pair you with one of our many bilingual support workers.
